The undersigned certifies (i) the information contained in this application and all supporting documents are true, correct, and complete and accurately represent the financial condition of the applicant on the date set forth below, and (ii) the credit being applied for is for business purposes and will not be used for personal, family or household purposes. The undersigned acknowledges the representations made in this certification will be relied upon by FAIRWINDS Credit Union (the lender) in its evaluation of this application. The undersigned will promptly notify the lender of any subsequent changes that affect the accuracy of the information provided in this application or any supporting document or adversely affect the financial condition of the undersigned. The undersigned recognizes and agrees acceptance of this application is not a commitment to lend; the undersigned may be required to submit additional information, and the lender may keep this application whether or not it is approved. The undersigned understands Title 18, Section 1014 of the U.S. Code sets forth criminal penalties for fraud or false statements to influence credit unions insured by the NCUA. The undersigned authorizes FAIRWINDS Credit Union to obtain a credit report, make all inquiries it deems necessary to verify the accuracy of information contained in this application or any supporting document and to determine the creditworthiness of the undersigned, and answer any inquiries about the undersigned’s credit experience with the lender. In addition, each individual signing below authorizes the lender to check his/her individual credit account and employment history and have a credit reporting agency prepare and provide to the lender a consumer credit report on such person.
